Does the typical patient age range of systemic sclerosis and lupus help you determine what is going on here?
Lupus typically affects women of child-bearing age. One complication of lupus is thrombosis of the placenta (e.g. associated with the antiphospholipid antibodies) resulting in placental infarctions and intrauterine fetal death. The gross image depicts a soldiery infarct (that is only visible upon cut section). Usually multiple infarcts of large areas of the placenta are required to compromise the fetus. Microscopically there is an organizing thrombus in the placental vasculature with an adjacent infarct.
