This is an example of cytomegalovirus (CMV) pneumonia. Grossly note the diffuse hemorrhagic parenchyma seen in diffuse alveolar damage. The micrograph shows a "dirty" acute inflammatory infiltrate surrounding numerous cells with viral inclusions (which classically look like "owl's eyes"). Most adults are latently infected with CMV. In heathy adults and children the primary infection is usually asymptomatic, but can reemerge with immunocompromise (e.g. AIDS or organ transplant). The subsequent infections can be quite serious, primarily affecting the lungs and gastrointestinal tract. Organ transplant patients can also get the virus from infected transplanted organs.
